儿童早期龋齿接受全身麻醉牙科修复后口腔健康相关生活质量的评价

Evaluation of Oral Health-related Quality of Life in Children with Early Childhood Caries after Dental Rehabilitation under General Anesthesia

摘要: 目的:评估全身麻醉下一次性牙科综合治疗对早期龋齿(early childhood caries,ECC)患儿口腔健康相关生活质量(oral health-related quality of life,OHRQoL)的改善效果及其影响因素。方法:选取80例3~6岁ECC患儿,在全身麻醉下完成一次性牙科治疗。使用中文版幼儿早期口腔健康影响量表(early childhood oral health impact scale,ECOHIS)于治疗前及治疗后30 d进行评估,分析OHRQoL的变化及其与性别、年龄、家庭收入、治疗方式等因素的关系。结果:治疗后ECOHIS总分由15.24±3.93降至3.05±1.44(P<0.001),效应量(ES)为1.23。女性、年龄<4岁、低收入家庭及接受拔牙治疗的患儿OHRQoL改善更为显著(P<0.05)。多元回归分析显示性别、年龄、家庭收入和是否拔牙是OHRQoL改善的独立预测因素(调整R2=0.412)。结论:全身麻醉下牙科治疗可显著改善ECC患儿的OHRQoL,尤其对女性、低龄、低收入及接受拔牙治疗的患儿效果更佳。

Abstract: Objective: To evaluate the improvement in oral health-related quality of life (OHRQoL) and its influencing factors in children with early childhood caries (ECC) after comprehensive dental treatment under general anesthesia. Methods: A total of 80 children aged 3-6 years with ECC underwent one-time dental rehabilitation under general anesthesia. The Chinese version of the Early Childhood Oral Health Impact Scale (ECOHIS) was used to assess OHRQoL before and 30 days after treatment. Changes in ECOHIS scores and their associations with gender, age, family income, and treatment type were analyzed. Results: The total ECOHIS score decreased significantly from 15.24±3.93 to 3.05±1.44 (P<0.001), with an effect size (ES) of 1.23. Greater improvements were observed in girls, children under 4 years, those from low-income families, and those who underwent tooth extraction (P<0.05). Multiple regression analysis identified gender, age, family income, and tooth extraction as independent predictors of OHRQoL improvement (adjusted R2=0.412). Conclusion: Dental treatment under general anesthesia significantly improves OHRQoL in children with ECC, particularly in girls, younger children, those from low-income families, and those receiving tooth extraction.
