时域参数定量个别正常牙合咀嚼相关肌肉表面激活状态

Quantitative Analysis of Surface Activation of Mastication-related Muscles in Individuals with Normal Occlusion Using Time-domain Parameters

摘要: 目的: 通过同步监测4对咀嚼相关肌肉,建立18~30岁个别正常牙合受试者口腔咀嚼系统时域参考范围。方法: 对69名18~30岁个别正常牙合受试者,同步监测双侧颞肌前束等4对咀嚼相关肌肉,建立5种下颌动作下的表面肌电图(surface electromyography, sEMG)参数及特异值参考范围。结果: 颞肌和咬肌为主要咀嚼肌,紧咬牙位时活动最强;二腹肌前腹于下颌姿势位活动明显以维持下颌稳定;胸锁乳突肌单侧咀嚼时不对称激活维持头颈稳定,证实其与咀嚼功能的协同作用。单侧咀嚼呈“咀嚼侧优势”,部分肌电参数存在性别差异(P<0.05)。结论: 本研究建立18~30岁个别正常牙合人群多肌群、多动作肌电参考范围,揭示咀嚼肌功能分工,为口腔咀嚼系统功能量化评估提供依据。

Abstract: Objective: To establish the time-domain reference range of the oral masticatory system for individuals with normal occlusion aged 18-30 years by synchronously monitoring four pairs of masticatory-related muscles. Methods: A total of 69 subjects aged 18-30 years with normal occlusion were included. Four pairs of masticatory-related muscles (including the anterior bundles of bilateral temporalis muscles) were monitored synchronously to establish the reference ranges of surface electromyography (sEMG) parameters and specific values under five mandibular movements. Results: The temporalis and masseter muscles were the main masticatory muscles, with the strongest activity in the maximal clenching position. The anterior belly of the digastric muscle showed obvious activity in the mandibular postural position to maintain mandibular stability. The sternocleidomastoid muscle was asymmetrically activated during unilateral mastication to maintain craniocervical stability, confirming its synergistic effect with masticatory function. Unilateral mastication presented a significant "chewing side dominance", and gender differences were observed in some EMG parameters (P<0.05). Conclusion: This study establishes the EMG reference ranges for multiple muscle groups and multiple movements in individuals with normal occlusion aged 18-30 years, clarifies the functional division of masticatory muscles, and provides a basis for the quantitative evaluation of oral masticatory system function.
