骨性扩弓器对上颌横向发育不足患者治疗效果的CBCT研究

CBCT Study on Therapeutic Effects of Skeletal Expander in Patients with Maxillary Transverse Hypoplasia

摘要: 目的:探究上颌骨性扩弓器治疗上颌横向发育不足的治疗效果。方法:选取2019~2024年武汉大学口腔医院24例患者,采用微种植体辅助上颌骨性扩弓治疗,通过CBCT分析治疗前后牙骨指标。由同一医生重复测量,经ICC系数验证一致性后,采用配对样本t检验等进行统计学分析。结果:扩弓成功率100%,前磨牙区及磨牙区牙槽骨宽度增加且颊侧倾斜(P<0.05)。上下颌相关牙齿水平距离增加、颊侧倾斜,上颌第一磨牙牙槽骨及牙齿颊侧倾斜更显著,部分指标差异有统计学意义(P<0.05)。结论:微种植体辅助上颌骨性扩弓骨性效果显著、成功率高,可有效扩展上颌骨,同时对上下颌牙齿产生明显牙性效应,上颌第一磨牙作为支抗牙作用突出。

Abstract: Objective: To investigate the therapeutic efficacy of maxillary skeletal expanders in the management of maxillary transverse deficiency (MTD). Methods: A total of 24 patients who received treatment at Wuhan University Stomatological Hospital between 2019 and 2024 were enrolled. All subjects underwent microimplant-assisted maxillary skeletal expansion (MIA-MSE), and cone-beam computed tomography (CBCT) data were acquired pre- and post-treatment to analyze dental and skeletal parameters. Duplicate measurements were performed by the same clinician, with interclass correlation coefficient (ICC) used to verify measurement consistency. Statistical analyses were conducted using paired samples t-test and other appropriate methods. Results: The overall success rate of expansion was 100%. Significant increases in alveolar bone width combined with buccal inclination were observed in the premolar and molar regions (P<0.05). For both maxillary and mandibular relevant teeth, increased horizontal spacing and buccal inclination were noted, with more pronounced buccal inclination of alveolar bone and teeth in the maxillary first molars. Several indicators exhibited statistically significant differences (P<0.05). Conclusion: Microimplant-assisted maxillary skeletal expansion achieves remarkable skeletal effects with a high success rate. It effectively expands the maxilla while exerting significant dental effects on both maxillary and mandibular dentition, with the maxillary first molar serving as a prominent anchor tooth.
