老年天然牙与三种口腔修复材料摩擦磨损学性能的实验研究

Comparative Study on Wear and Friction Properties of Natural Aged Teeth with Three Dental Restorative Materials

摘要: 目的:对老年天然牙及口腔修复材料的摩擦磨损学性能进行比较,并进行摩擦磨损机制的分析,探讨材料与老年天然牙耐磨损性能的匹配情况,为临床修复材料的选择提供实验依据。方法:使用体外摩擦磨损试验机,模拟口腔生理环境,将临床收集的60岁以上老人完整的天然牙,纯钛,氧化锆瓷块,玻璃陶瓷进行体外二体摩擦磨损实验。采用扫描电镜观察表面磨损形貌,显微硬度仪测试表面硬度,电子天平得出磨损量。结果:测试的5种材料的磨损量,即3种口腔修复材料,老年天然牙牙釉质,老年天然牙牙本质的磨损量之间均有统计学差异(P<0.05),其中纯钛磨损量与牙釉质最接近,牙本质磨损量显著高于牙釉质和3种修复材料。老年天然牙牙釉质的磨损机制主要是疲劳磨损,牙本质主要是磨粒磨损,纯钛主要是黏着磨损和磨粒磨损,玻璃陶瓷主要是磨粒磨损和黏着磨损,氧化锆主要是磨粒磨损。结论:纯钛耐磨损性能与老年天然牙牙釉质较相匹配,老年天然牙牙釉质耐磨损性能明显优于牙本质,在临床上,应根据对颌天然牙磨耗情况,合理选择修复材料,避免造成牙釉质尤其牙本质的过度磨耗。

Abstract: Objective: To compare the friction and wear properties of natural aged teeth with dental restorative materials and to analyze the friction and wear mechanism. Methods: In vitro friction and wear tests were performed between natural teeth collected from patients over 60 years old and pure titanium, zirconia, and glass-ceramic. The surface wear morphology was observed by scanning electron microscope, the surface hardness was measured by micro-hardness tester, and the wear amount was obtained by electronic balance. Results: There were significant differences between three materials and the enamel and dentin (P<0.05). The wear of pure titanium was the closest to that of enamel, and the wear of dentin was significantly higher than that of enamel and three kinds of restorative materials. The wear mechanism of the aged natural tooth enamel was mainly fatigue wear, whereas the dentin was mainly exposed to abrasive wear. Pure titanium was mainly exposed to adhesive and abrasive wear. Abrasive and adhesive wear mainly occurred with glass-ceramic while zirconia was largely exposed to abrasive wear. Conclusion: The wear resistance of pure titanium matches the enamel of natural teeth in old age. The wear resistance of natural teeth enamel is much better than that of dentin. In clinic, according to the wear condition of natural teeth in jaw, it is suggested to rationally select restorative materials, avoiding the excessive wear of natural teeth, especially dentin.
