增龄性因素对固定矫正患者初粘后疼痛的影响

Effect of Age-related Factors on Pain after Initial Adhesion in Patients with Fixed Appliance

摘要: 目的: 初步探查年龄因素对于正畸患者初戴托槽后疼痛水平的影响,为正畸疼痛个性化防治提供临床证据。方法: 共纳入青少年(11~14岁)、成年(18~25岁)唇侧固定矫治患者各18例,选取初次粘接托槽后7个不同时间点(加力即刻、4 h、12 h、1 d、2 d、3 d、7 d)进行视觉模拟评分(visual analog scale,VAS)评估年龄因素对正畸疼痛水平的影响。结果: 对初次加力后VAS基于广义估计方程进行统计分析,加力时间主效应显著(F=94.294,P<0.01),年龄和加力时间交互效应显著(F=54.114,P<0.01),初次加力后4 h、2 d、3 d增龄性效应显著(4 h时P<0.01;2 d时P<0.05;3 d时P<0.01)。结论: 戴固定矫治器的青少年患者对机械力反应迅速而敏感,成年组患者对机械力施加后的炎性疼痛反应迟缓但更为持久。

Abstract: Objective: To explore the influence of age on the pain level of orthodontic patients after initial bracket wearing. Methods: A total of 18 adolescents (11-14 years old) and 18 adults (18-25 years old) with straight wire appliance were enrolled in this study. Visual analog scale (VAS) was used to evaluate the effect of age on orthodontic pain at 7 different time points after the first bracket bonding (immediately, 4 h, 12 h, 1 d, 2 d, 3 d, and 7 d). Results: The time effect was significant (F=94.294,P<0.01), the main effect of age was not significant (P>0.05), and the interaction effect of age and time was significant (F=54.114,P<0.01). The age-related effects were significant at 4 h, 2 d, and 3 d after the first loading (P<0.01 at 4 h, P<0.05 at 2 d, and P<0.01 at 3 d). Conclusion: Adolescent patients with fixed appliances respond quickly and sensitively to mechanical force, while adult patients respond slowly but more persistently to inflammatory pain after mechanical force.
