婴儿单侧完全性唇腭裂唇鼻体表特点的测量研究

摘要: 目的:研究单侧完全性唇腭裂的唇鼻体表特点,为适龄婴儿手术治疗及治疗后效果的评估提供理论参考和指导。方法:按纳入标准随机选取92例单侧完全性唇腭裂患儿,采用直接测量法对其唇鼻体表进行测量,与正常对照组比较。结果:获得单侧完全性唇腭裂婴儿和正常婴儿唇鼻体表的28项数据的基本均值和标准差。单侧完全性唇腭裂的数据与对照组比较:除去患侧高点上唇红厚、健侧唇高及健侧鼻孔底宽无统计学差异外,其余数据均有显著性统计学差异(P<0.01)。单侧完全性唇腭裂数据间的相关分析发现多项数据间存在相关性,有统计学意义(P<0.05)。结论:单侧完全性唇腭裂具有复杂的唇、鼻畸形,在三维空间上形成一个畸形的整体,既表现有组织的缺损又存在组织的移位,各项测量指标的差异很大但在同一个体的成对指标间却具有明显的正相关性。

Abstract: Objective: To provide a theoretical reference for the clinical treatment and efficacy assessment of unilateral complete cleft lip by mastering the characteristics of the lip nasal soft tissue profiles of the infants with unilateral complete cleft lip and palate (UCLP). Methods: 92 infants with UCLP were randomly selected, measured the lip nasal soft tissue profiles directly and compared with normal control group. Results: The mean value and standard deviation of 28 facial basic measurement indexes were obtained from normal infants and infants with UCLP. We compared the measurements of UCLP group with normal group. Three indexes including lateral upper vermilion heigh in the affected side, upper lip height in the unaffected side, the nose bottom width in the unaffected side, were not significant different. The other indexes were statistically significant different (P<0.01). The correlation analysis of measurements in UCLP group showed that there were correlations and statistically significance among several indexes. Conclusion: UCLP was a complex lip-nasal deformity, formed a whole in three dimension. It displayed tissue defect and displacement. The measurement indexes differed greatly, but there was a obvious positive correlation between paired indexes of the same individual.
