原位点种植体再植的存留率及相关影响因素的回顾性分析

Retrospective Analysis of Survival Rate and Related Factors of Implant Replantation at Previous Failed Site

摘要: 目的: 评估分析失败再植的临床预后及相关影响因素。方法: 按照纳入和排除标准严格筛选2016年1月~2021年6月于南京大学医学院附属口腔医院种植科因牙列缺损行种植体再植的患者,记录患者性别、年龄、有无系统病史、是否吸烟、种植位点、种植系统、种植体长度和直径、骨增量与否、再植时机、首次和再次种植时间、失败时间及原因。采用χ2检验及Cox回归分析再植种植体的存留率以及可能存在的影响因素。结果: 最终纳入224例患者,共234颗种植体,其中11颗种植体失败,再植存留率为95.3%。早期失败植体多于晚期失败。通过χ2检验及cox回归分析发现年龄和糖尿病史可显著影响再植种植体存留,吸烟患者的再植种植体失败风险更高。结论: 种植体原位再植仍可获得较高的临床存留率,且患者的年龄、糖尿病史、吸烟史是显著的影响因素,临床需给予高度重视。

Abstract: Objective: To evaluate the clinical prognosis and related factors of failed implant replantation. Methods: According to the inclusion and exclusion criteria, patients who underwent implant replantation due to dental defect in a Grade-A dental hospital in Jiangsu from 2016.1 to 2021.6 were strictly screened. The patient's gender, age, systematic medical history, smoking status, implant site, implant system, implant length and diameter, bone increment or not, replantation time, first and second implantation time, and failure time and reason were recorded. Chi-square test and Cox regression were used to analyze the retention rate of replanted implants and the possible influencing factors. Results: A total of 224 patients and 234 implants were included in the study. Eleven implants failed secondly and the replantation retention rate was 95.3%. There were more early failures than late failures. Chi-square test and cox regression analysis showed that age and history of diabetes significantly affected the retention of replantation implants, and smoking patients had a higher risk of implant failure. Conclusion: Implant replantation can still obtain a high clinical survival rate. The patient's age, diabetes history, and smoking history are significant influencing factors.
