成人Ⅱ类骨面型上颌骨后牙区骨皮质厚度和安全性的锥形束CT研究

Study on Cortical Bone Thickness and Safety in Maxillary Posterior Interdental Area in Adult of Skeletal Class Ⅱ Malocclusion by Cone Beam CT

摘要: 目的:通过对安氏Ⅱ类骨面型患者锥形束CT(CBCT)分析,测量上颌第二前磨牙与第一磨牙区域不同高度不同角度下骨皮质厚度和骨质厚度,为临床上Ⅱ类 骨面型患者植入微种植钉提供参考。方法:对30名成人患者(18~40岁)的上颌骨进行CBCT扫描和三维重建,分别测量上颌第二前磨牙和第一磨牙间距牙槽嵴顶4 mm、6 mm、8 mm三个位点以45°、60°、90°角度植入时的有效骨皮质和骨质厚度,测量结果应用SPSS17.0软件包进行统计学分析。结果:1)同一高度与骨面呈45°植入微种植钉时得到的骨内皮质最厚,90°最薄(P<0.05);2) 同一角度植入微种植钉时,植入部位越高得到的骨皮质厚度越厚(P<0.05)。结论:安氏Ⅱ类患者后牙区距离牙槽嵴越远骨皮质厚度越大,且同一植入高度下,以倾斜角度45°植入微种植体较60°,90°可获得更佳的骨皮质厚度。

Abstract: Objective: To provide some references of using micro-implant anchorage in adult of skeletal Class Ⅱ malocclusion by measuring the thickness of buccal cortical bone and the buccolingual bone of different insertion angle type in the maxillary second premolar and first molar area by cone-beam CT (CBCT). Methods: The maxillary posterior area of 30 adult patients (18<year<40) were scanned and reconstructed by CBCT. The cortical bone and the bone thickness were measured at 4, 6, and 8mm from the alveolar crest with 45°, 60°, 90° angle in maxillary posterior interdental area between first molar and second premolar. The data were analyzed statistically. Results: The cortical thickness of buccal bone in maxillary posterior interdental area was thicker at 8mm than 6mm. 4mm was the thinnest with the same insertion angle (P<0.05). The cortical bone thickness was the thickest with 45° insertion angle and the thinnest with 90° insertion angle at the same height level (P<0.05). Conclusion: The study suggested that the conical bone thickness is thicker when the implantation location was more away from the alveolar crest, and the insertion angle with 45° also can provides best cortical bone thickness at same height level.
