99Tcm-RBC SPECT显像在诊断颌面部脉管畸形的应用价值

Application of 99Tcm-RBC SPECT in Diagnosis of Maxillofacial Vascular Malformations

摘要: 目的: 探讨单光发射型计算机断层(single-photo emission computed tomography)血池显像诊断颌面部脉管畸形的价值。方法: 对40例临床上怀疑颌面部脉管畸形患者进行颌面部核素扫描,分别进行SPECT血池显像,以99Tcm-RBC为显像剂,观察不同类型脉管畸形血池显像情况,诊断结果与MRI比较进行χ2检验。结果: 99Tcm-RBC血池显像对颌面部静脉畸形诊断敏感度、特异性、准确性分别是97.50%%、100.00%、100.00%。99Tcm-RBC血池显像40例患者其中39例阳性(97.50%,39/40);1例显示假阴性(2.50%,1/40);MRI对颌面部静脉畸形诊断其中33例阳性(82.50%,33/40),7例为阴性(7.50%,7/40),99Tcm-RBC血池显像与MRI影像学对比检查经χ2检验有统计学意义(χ2=0.028,P<0.05)。不同类型脉管畸形血池显像出现在不同时相。结论: 99Tcm-RBC血池显像诊断颌面部脉管畸形有良好的应用价值。

Abstract: Objective: To evaluate the application of 99Tcm-RBC blood pool scintigraphy in diagnosis of maxillofacial vascular malformations. Methods: Forty patients with clinically suspected maxillofacial vein malformations were examined by 99Tcm-RBC blood pool scintigraphy and the results were compared with MRI. The chi-square test was used to compare the significance. Results: The sensitivity, specificity, and accuracy of 99Tcm-RBC blood pool scintigraphy in diagnosis of maxillofacial vein malformations were 97.50%, 100.00%, and 100.00%, respectively. Thirty-nine of 40 patients who had maxillofacial vein malformations showed positive images (97.50%), and 1 patients showed false positive images (2.50%); 33 of 40 patients by MRI who had malignant tumors showed positive image (82.50%), and 7 patients showed negative image (7.50%). There was a significant difference between two groups by the chi-squre test (P<0.05). Conclusion: It is suggested that 9Tcm-RBC blood pool scintigraphy can be used in diagnosis of maxillofacial vein malformations.
