6例下颌骨Garrè's骨髓炎临床分析

Clinical Characteristic of 6 Patients with Garrè's Osteomyelitis

摘要: 目的: 本文通过分析颌骨Garrè's骨髓炎患者的一般情况,临床特征,感染来源,分布及治疗,为颌骨Garrè's骨髓炎的防治,诊断及治疗提供依据。方法: 收集2015年 2月~2017 年10月期间新疆医科大学第一附属医院口腔颌面肿瘤外科收治的6例Garrè's颌骨骨髓炎的患者病例资料,对患者的一般情况、临床特征、感染来源,分布及治疗进行分析。结果: 6例颌骨Garrè's骨髓炎患者中,男女各3例,平均年龄11.6岁,住院天数8 d,平均病程99 d。 4例患者为非牙源性感染(67% );2例患者为牙源性感染(33%);Garrè's颌骨骨髓炎影像学特点为致密性骨硬化伴骨膜新骨形成。结论: Garrè's颌骨骨髓炎是一种常累及单侧下颌骨的特殊骨质紊乱性慢性骨髓炎,病因与牙源性感染,内分泌及外伤有关。手术治疗辅助抗炎治疗是Garrè's颌骨骨髓炎的最佳治疗方案。

Abstract: Objective: To provide a basis for the prevention, diagnosis, and treatment of Garrè's osteomyelitis in the jaw by analyzing the general condition, clinical features, source, distribution, and treatment of patients with Garrè's osteomyelitis. Methods: The clinical data of 6 patients with Garrè's osteomyelitis treated in the Department of Oral and Maxillofacial Surgery of the First Affiliated Hospital of Xinjiang Medical University were analyzed from May 2013 to May 2015, including the patient's general condition, clinical characteristics, source of infection, distribution, and treatment. Results: Of the 6 patients with Garrè's osteomyelitis of the jaw, 3 were male and 3 were female, with an average age of 11.6 years, 8 days of hospitalization, and an average duration of 99 days. 4 patients were non-dental infections (67%). 2 patients had odontogenic infections (33%). The jaws of Garrè's osteomyelitis often involved the mandible. It had facial asymmetry and local swelling. The imaging feature of Garrè's jaw osteomyelitis was dense osteosclerosis with periosteal new bone formation. Conclusion: Garrè's osteomyelitis is a special osteoporotic chronic osteomyelitis,which often involved the unilateral mandible. The etiology is related to odontogenic infection, endocrine, and trauma. Surgical treatment of adjuvant anti-inflammatory treatment is the best treatment for Garrè's osteomyelitis.
