角化龈增量对维护牙周炎患者种植体稳定性的临床研究

Clinical Study of Keratinized Gingiva Augmentation in Maintaining Implant Stability with Chronic Periodontitis

摘要: 目的:评估根向复位瓣联合游离龈移植术(FGG)重建慢性牙周炎患者种植体角化龈的临床效果,以期为临床提供参考。方法:选取慢性牙周炎患者种植后出现种植体周角化龈不足的患者30例,随机分为FGG组和对照组,每组各含19个种植体,观察比较两组种植体周围组织临床指标的变化。结果:在观察期间两组种植体功能良好,FGG组部分测量指标[角化龈宽度(KTW)、牙龈指数(GI)、改良菌斑指数(mPI)、龈缘退缩(GR)、边缘骨吸收(MBL)]较基线改善,差异均有统计学意义(P<0.05);6个月随访,两组种植体周围组织状况(KTW、GI、PI、GR)相比,FGG组改善明显,差异均有统计学意义(P<0.05)。结论:根向复位瓣联合游离龈移植可以增加牙周炎患者种植体周围角化龈的宽度,在短期内能维护种植体周健康和稳定种植体周围骨水平。

Abstract: Objective: To evaluate the clinical effect of apically positioned flap combined with free gingival graft on the reconstruction of implant keratinized gingiva in patients with chronic periodontitis.Methods: Thirty chronic periodontitis patients with insufficient keratinized gingiva around implants were selected and randomly divided into FGG group and control group, with 19 implants in each group. The clinical parameters, including keratinized tissue width (KTW), peri-implant probing depth (PPD), gingival index (GI), modified plaque index (mPI), mucosal recession (MR), and marginal bone loss (MBL) were compared between two groups at baseline, 3 months, and 6 months after treatment.Results: During the period, the implant function of two groups was good, and some measurement indicators (KTW, GI, mPI, GR, MBL) in the FGG group were improved compared with the baseline (P<0.05). After 6 months of follow-up, the conditions of peri-implant tissues (KTW, GI, PI, GR) in the FGG group were significantly improved compared with those in the control group (P<0.05).Conclusion: APF combined with FGG is a practical method to reconstruct the keratinized gingiva of implants in patients with periodontitis. It can maintain the health and stabilize the bone level around the implant in short term.
