定制化舌侧前牙托槽转矩预设对牙齿移动影响的三维有限元分析

Influence of Torque Prescription in Customized Lingual Brackets on Anterior Tooth Movement: Three-dimensional Finite Element Analysis

摘要: 目的: 研究上前牙舌侧托槽转矩预设对前后牙齿三维位移的影响。方法: 构建上颌个性化舌侧矫治的三维有限元模型,分别预设A、B两组前牙托槽转矩,A组:中切牙、侧切牙、尖牙分别设为+13°、+10°、+7°;B组:相应牙齿设置为+20°、+13°、+9°。在没有牵引力作用下,观察两组牙齿初始位移的变化。结果: 0.017英寸×0.025英寸弓丝就位后,A、B两组前牙均表现为冠唇向倾斜伴龈向压低趋势,后牙区普遍表现为冠舌倾、压低及近中位移,且舌倾程度从第二前磨牙向第二磨牙递增,其中第二磨牙出现腭尖压低、颊尖伸长的特殊位移趋势.除尖牙冠唇向位移在B组略有减小外,B组其余牙齿位移量均大于A组。结论: 前牙托槽预设冠唇向转矩有助于改善前牙转矩控制,但需注意防范后牙区可能出现的冠舌倾、颊尖伸长等不良反应,尤其是第二磨牙;同时应关注后牙宽度的维持,以免影响尖牙区转矩表达的有效性。

Abstract: Objective: To investigate the effects of torque prescription in maxillary anterior lingual brackets on three-dimensional tooth displacement of anterior and posterior teeth. Methods: A three-dimensional finite element model of maxillary dentition with customized lingual orthodontics was established. Two groups of anterior bracket torque prescriptions were set: Group A, central incisors, lateral incisors, and canines were set at +13°, +10°, and +7°, respectively; Group B, the corresponding teeth were set at +20°, +13°, and +9°, respectively. Without traction forces, the initial tooth displacement in both groups was observed. Results: After the engagement of the 0.017×0.025-inch archwire, the anterior teeth in both groups exhibited crown labial inclination with an intrusive tendency, while the posterior teeth generally showed crown lingual inclination, intrusion, and mesial displacement. The degree of lingual inclination increased from the second premolar to the second molar. Notably, the second molar exhibited a specific displacement trend: palatal cusp intrusion and buccal cusp extrusion. Except for a slight decrease in the crown labial displacement of the canines in Group B, all other tooth displacements in Group B were greater than those in Group A. Conclusions: Presetting crown labial torque in anterior brackets improves anterior torque control, while caution is required to prevent side effects in the posterior region, such as crown lingual inclination and buccal cusp extrusion (especially in the second molars), as well as to maintain the posterior arch width for effective torque expression in the canine region.
