一种数字化全口义齿的临床应用初步探讨

Primary Clinical Application of 3D Printing Complete Dentures

摘要: 目的: 探讨3D扫描和3D打印技术在无牙颌修复中的应用。方法: 选取自2018年3月1日~2020年5月1日来我院就诊无牙颌病例30例(Atwood分级1~4级),随机分为传统义齿组和3D打印义齿组,每组15例,传统义齿组按照传统规范制作全口义齿,3D打印义齿组通过3D扫描、数字设计、3D打印结合复制义齿技术制作全口义齿。研究采用单盲法。分别在义齿戴用即时,1个月,3个月,6个月使用义齿满意度视觉模拟评分表(visual analogue scale,VAS)进行疗效评估,使用SPSS 22.0对结果进行统计分析。结果: 两组义齿语音功能,咀嚼功能和舒适性随戴用时间逐步提高,戴用3个月后,两组义齿美观度,语音功能、咀嚼功能、固位功能、舒适性均能达到患者满意,VAS评分无显著性差异(P>0.05)。相比传统义齿,3D打印义齿复诊次数明显减少。结论: 3D打印数字化全口义齿能够快速实现无牙颌的修复,满足患者美观和功能的需要。

Abstract: Objective: To primary explore the application of 3D scanning and 3D printing technique in the restoration of edentulous patient. Methods: 30 edentulous cases (Atwood class 1-4) visited our hospital from 3/1/2018 to 5/1/2020 were selected. The cases were randomly divided into 2 groups: the traditional complete denture group (Group A) and the 3D printing complete denture group (Group B). Each group had 15 cases. Group A used traditional method to manufacture the complete denture, and Group B used 3D scanning, CAD (Computer-aided design), 3D printing, and duplicate denture technique to manufacture the dentures. The patients' satisfaction was measured using the visual analogue scale (VAS) at 4 time points: immediately after denture delivery, 1 month, 3 months, and 6 months following denture delivery. Results: The ability to speak and chew and the comfort of two groups of dentures were gradually improved in the first three time points. The VAS scores increased to a satisfy level after 3 months. The esthetics and stability of two groups of dentures scored high since the initial delivery. The VAS scores of two groups on esthetics, ability to speak, ability to chew, stability, and comfort had no statistic difference (P>0.05) on each time point. The visiting times of 3D printing complete denture were significantly decreased in comparison to traditional complete denture. Conclusion: 3D printing complete denture can restore the edentulous patients rapidly, so as to meet the patients’ demanding of esthetic and function.
