不同正畸治疗措施对错牙合畸形患者龈沟液MMP-2、TIMP-2水平的影响分析

Effect of Orthodontic Treatment on Levels of MMP-2 and TIMP-2 in Gingival Crevicular Fluids of Malocclusion Patients.

摘要: 目的:分析不同正畸治疗措施对错牙合畸形患者龈沟液基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)、金属蛋白酶组织抑制物-2(tissue inhibitors of metalloproteinases-2,TIMP-2)水平的影响。方法:选取医院收治的错牙合畸形患者60例,按照随机数字表法分为A组、B组、C组和D组,每组各15例。所有患者均接受正畸治疗,4组患者分别给予0、50、150 g和250 g的远中移动初始力。比较4组患者治疗前及治疗1、2、3、4周龈沟液MMP-2、TIMP-2水平及其比值。结果:治疗1、2、3、4周后,B组、C组沟液MMP-2、TIMP-2水平显著高于A组和D组,且C组MMP-2、TIMP-2水平显著高于B组,差异有统计学意义(P<0.05);B组和C组龈沟液MMP-2/TIMP-2比值治疗1周后明显升高,治疗2、3、4周后呈现小幅度降低趋势;A组和D组治疗前后沟液MMP-2、TIMP-2水平比较差异均无统计学意义。结论:龈沟液中MMP-2、TIMP-2水平与正畸力的大小以及持续时间有关,而且龈沟液MMP-2、TIMP-2动态水平变化可作为分析正畸牙加力的重要参考指标。

Abstract: Objective: To analyze the effect of orthodontic treatment on the levels of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of tissue inhibitors of metalloproteinases-2 (TIMP-2) in gingival crevicular fluids of patients with malocclusion. Methods: Sixty cases of malocclusion treated in the hospital were selected, which were divided into group A, B, C, and D according to the random number table method (n=15). All patients were treated with orthodontic treatment, who were given 0, 50, 150, and 250 g of distal movement initial force, respectively. The level and ratio of MMP-2 and TIMP-2 in gingival crevicular fluids before and 1, 2, 3 and 4 weeks after treatment were compared. Results: After treatment for 1, 2, 3, and 4 weeks, the levels of MMP-2 and TIMP-2 in group B and C were significantly higher than those in group A and D. And the levels of MMP-2 and TIMP-2 in group C were significantly higher than those in group B (P<0.05). The MMP-2/TIMP-2 ratios of gingival crevicular fluid in group B and C were significantly increased after 1 week, however, little decreased after 2, 3, and 4 weeks. There were no significant difference in the levels of MMP-2 and TIMP-2 in group A and D before and after treatment. Conclusion: The levels of MMP-2 and TIMP-2 in gingival crevicular fluid are related to the size and duration of orthodontic force, and the dynamic levels of MMP-2 and TIMP-2 in gingival crevicular fluid can be used as an important reference index to analyze the orthodontic tooth augmentation.
