口腔鳞状细胞癌组织TGF-β1表达和临床病理学参数对预后的影响

Significance of TGF-β1 Expression and Clinicopathological Characteristics on Prognosis for Oral Squamous Cell Carcinoma Patients.

摘要: 目的: 探讨转化生长因子β1(TGF-β1)在口腔鳞癌肿瘤中心和浸润前沿处的表达及其预后意义。方法: 收集104例手术切除的口腔鳞癌组织,采用免疫组化法检测肿瘤中心和浸润前沿处TGF-β1的表达。结果: TGF-β1在肿瘤中心和浸润前沿处的阳性表达率分别为93.3%、89.4%,无显著差异(P>0.05)。χ2检验显示肿瘤中心和浸润前沿处TGF-β1高表达与高等级TNM分期、肿瘤浸润深度≥5 mm、发生神经侵犯或复发显著正相关(均P<0.05)。Kaplan-Meier分析显示TGF-β1在肿瘤中心和浸润前沿处高表达与总生存率、无复发生存率和无病生存率降低显著相关(均P<0.05)。Cox多因素分析显示TGF-β1在肿瘤中心处高表达与患者总生存率和无病生存率降低显著相关(均P<0.05)。结论: TGF-β1在肿瘤中心处高表达可能是口腔鳞癌患者总生存率和无病生存率降低的独立影响因素。

Abstract: Objective: To investigate transforming growth factor beta1 (TGF-β1) expression at tumor center and invasive tumor front and the prognostic value in oral squamous cell carcinoma (OSCC). Methods: Paraffin-embedded sections of 104 patients with primary OSCC were collected at Nanjing Stomatological Hospital. The sections were stained by immunohistochemistry (IHC) and focused on regions at tumor center (TC) and invasive tumor front (ITF). Results: Positive expression rates of TGF-β1 between TC and ITF were comparable (P>0.05). Moreover, TGF-β1 expression at TC and ITF positively associated with TNM stages, DOI, nerve invasion and recurrence (P<0.05). Kaplan-Meier analysis showed that high expression of TGF-β1 no matter at TC or ITF was significantly correlated with lower overall survival, recurrence-free survival, and disease-free survival (P<0.05). Notably, Cox multivariate analysis indicated that high expression of TGF-β1 at TC correlated to low overall survival in OSCC patients. Conclusion: TGF-β1 at TC could be an independent prognostic factor for overall survival in OSCC patients.
