槲皮素基于TLR4/NF-κB通路对种植体周围炎大鼠的抗炎及骨保护作用

Anti-inflammatory and Bone Protective Effects of Quercetin on Peri-implantitis in Rats via TLR4/NF-κB Pathway

摘要: 目的:本研究以Toll样受体4(Toll-like receptor 4,TLR4)/核因子-κB(nuclear factor-κB,NF-κB)通路为切入点,探讨槲皮素(quercetin,QU)对种植体周围炎(peri-implantitis,PI)大鼠的抗炎及骨保护作用。方法:将36只大鼠随机分成Ctrl组、PI组和QU组,检测大鼠种植体炎症指标,测量种植体周围骨高度和骨密度,苏木精-伊红(hematoxylin-eosin, HE)染色观察种植体周围的牙龈组织病理变化。酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测牙龈组织中肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素(interleukin,IL)-6和单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)的含量,以及龈沟液中基质金属蛋白酶-8(matrix metalloproteinase-8,MMP-8)和Ⅱ型胶原(collagen type Ⅱ,COL-Ⅱ)的含量。Western blot检测骨代谢和TLR4/NF-κB信号通路相关蛋白的表达水平。结果:与Ctrl组比较,PI组大鼠种植体近颊、远颊、近舌和远舌处软组织的袋深度(pocket probing depth,PPD)、出血指数(bleeding index,BI)和牙龈指数(gingival index,GI)值,TNF-α、IL-6、MCP-1、MMP-8和COL-Ⅱ水平,核因子κB受体活化因子配体(receptor activator of nuclear factor-κB ligand,RANKL)、TLR4和p-NF-κB p65/NF-κB p65蛋白表达水平显著增加(P<0.05);种植体的骨高度以及骨密度、骨保护素(osteoprotegerin,OPG)蛋白表达水平显著降低(P<0.05)。与PI组比较,QU组大鼠种植体近颊、远颊、近舌和远舌处软组织的PPD以及BI和GI值,TNF-α、IL-6、MCP-1、MMP-8和COL-Ⅱ水平,RANKL、TLR4和p-NF-κB p65/NF-κB p65蛋白表达水平显著降低(P<0.05);种植体的骨高度以及骨密度,OPG蛋白表达水平显著增加(P<0.05)。结论:槲皮素可有效减轻PI大鼠的炎症反应及骨吸收,其作用机制可能与抑制TLR4/NF-κB信号通路有关。

Abstract: Objective: To explore the anti-inflammatory and bone protective effects of quercetin (QU) on peri-implantitis (PI) rats. Methods: Thirty-six Sprague-Dawley (SD) rats were randomly allocated to three groups: the control group (Ctrl group), the peri-implantitis group (PI group), and the treatment group (QU group). The inflammation indexes of implant in rats were detected. The bone height and bone density around the implant were measured. HE staining was employed to examine the pathological alterations of the gingival tissue surrounding the implant. The contents of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and monocyte chemoattractant protein-1 (MCP-1) in gingival tissue and the contents of matrix metalloproteinase-8 (MMP-8) and collagen type Ⅱ (COL-Ⅱ) in gingival crevicular fluid were detected by enzyme-linked immunosorbent assay (ELISA). Western blotting was used to detect the expression levels of bone metabolism and TLR4/NF-κB signaling pathway-related proteins. Results: Compared with the Ctrl group, the PPD, BI and GI values, TNF-α, IL-6, MCP-1, MMP-8 and COL-Ⅱ levels, receptor activator of nuclear factor-κB ligand (RANKL), TLR4 and p-NF-κB p65/NF-κB p65 protein expression levels were significantly increased in the PI group (P<0.05). Notably, significant reductions (P<0.05) were observed in the bone height and bone mineral density on the distal, proximal, buccal, and palatal aspects of the implant. Additionally, the expression level of the OPG protein exhibited a substantial decrease. When compared to the PI group, the QU group demonstrated significant decreases (P<0.05) in multiple parameters. Specifically, the pocket probing depth (PPD), bleeding index (BI), and gingival index (GI) values were notably lower. Additionally, the levels of TNF-α, IL-6, MCP-1, MMP-8 and COL-Ⅱ were significantly reduced. The protein expression levels of receptor activator of RANKL, TLR4, and the p-NF-κB p65/NF-κB p65 were also substantially decreased in the QU group.The bone height and bone mineral density of the distal, proximal, buccal and palatal sides of the implant and the expression level of OPG protein exhibited a notable and significant increase (P<0.05). Conclusion: Quercetin can effectively reduce the inflammatory response and bone resorption in rats with peri-implantitis, and its mechanism may be related to the inhibition of TLR4/NF-κB signaling pathway.
