光动力疗法在口腔潜在恶性疾患中的应用进展

摘要: 口腔潜在恶性疾患(oral potentially malignant disorders,OPMDs)是以口腔黏膜异常变化为特征、具有致癌潜力的一组黏膜疾病,其临床治疗旨在缓解症状、消除病损、阻止癌变。传统手术与药物治疗存在创伤大、不良反应多、患者依从性差等局限性,光动力疗法(photodynamic therapy,PDT)作为一种微创、靶向性治疗手段,在OPMDs的临床管理中展现出独特优势。本文旨在综述PDT在口腔白斑、口腔扁平苔藓、口腔黏膜下纤维化等主要OPMDs亚型中的临床应用进展,评估其疗效与安全性,并探讨当前面临的挑战与未来发展方向,以期为该疗法的规范化应用和未来研究提供参考。

Abstract: Oral potentially malignant disorders (OPMDs) are a group of mucosal diseases characterized by abnormal changes of the oral mucosa with potential risk of malignant transformation. Clinical treatment aims to alleviate symptoms, eliminate lesions, and prevent cancer development. Conventional methods, such as surgery and pharmacotherapy, are associated with limitations including invasiveness, significant side effects, and poor patient compliance. Photodynamic therapy (PDT), as a minimally invasive and precisely targeted treatment modality, has demonstrated promising outcomes in the management of OPMDs. This review summarizes recent advances in the clinical application of PDT for major OPMD subtypes, including oral leukoplakia, oral lichen planus, and oral submucous fibrosis. It evaluates the efficacy and safety of PDT, discusses current challenges, and suggests future directions, with the goal of providing a reference for standardizing PDT protocols and guiding further research.
