Correlation between Disc Perforation and Condylar Bone Changes Based on Temporomandibular Joint Arthrography by CBCT

摘要: 目的: 探讨颞下颌关节盘穿孔与髁突骨质改变的相关性。方法: 随机选取224例于我院因颞下颌关节紊乱病行CBCT下颞下颌关节上腔造影患者,根据造影结果,将患者分为4组,分别为关节盘穿孔伴髁突骨质改变组、关节盘穿孔而髁突骨质未改变组及关节盘未穿孔且髁突骨质未改变组、关节盘未穿孔而髁突骨质有改变组,探讨颞下颌关节盘穿孔与髁突骨质改变的相关性。结果: 颞下颌关节盘穿孔与髁突骨质改变有显著相关性(OR=21.28,P<0.05)。其中在不同类型的髁突骨质改变中,髁突骨质增生与关节盘穿孔相关性最大(OR=4.22,P<0.05)。结论: 颞下颌关节盘穿孔与髁突骨质改变有显著相关性,且髁突骨质增生最易伴发关节盘穿孔。但关节盘穿孔与髁突骨质改变发生的先后顺序还有待进一步研究。

Abstract: Objective: To investigate the correlation between temporomandibular joint disc perforation and condylar bone changes. Methods: A total of 224 patients with temporomandibular joint arthrography were taken CBCT in our hospital for temporomandibular joint disorders. According to the arthrography results, the patients were divided into four groups, which were disc perforation with condylar bone changed group, disc perforation and condylar bone unchanged group, disc unperforated and condylar bone unchanged group, articular disc not perforated and condylar bone changed group. We explored the relevance between disc perforation and condylar bone changes. Results: There was a significant correlation between temporomandibular joint disc perforation and condylar bone changes (OR=21.28,P<0.05). Among the eight types of condylar bone changes, condylar hyperosteogeny was the most relevant to disc perforation(OR=4.22,P<0.05). Conclusion: There is a significant correlation between temporomandibular joint disc perforation and condylar bone changes. Condylar hyperosteogeny is most likely to be associated with perforation, but the order of perforation of the disc and condylar bone changes remains to be further studied.
