Abstract: With the innovation and development of clear aligner therapy, its application in treating complex extraction cases was increased in clinical settings. However, due to the material properties of clear aligners and issues in treatment plan design, the "roller-coaster effect" often occurs during the retraction of anterior teeth and space closure. Thus, reducing the roller-coaster effect when using clear aligners has become a key focus and challenge for clinicians. This paper explores the biomechanical mechanisms of the roller-coaster effect in clear aligner therapy for premolar extraction cases and reviews the progress in its application.
