Abstract: Palatal radicular groove is a developmental anomaly on the root surface, which is difficult to keep clean and thus prone to localized periodontal tissue destruction. This case report describes the treatment of a maxillary lateral incisor with severe localized alveolar bone resorption caused by a palatal radicular groove. The condition was managed surgically using guided tissue regeneration (GTR) combined with bone grafting. At the one-year follow-up, significant improvement in clinical attachment loss and satisfactory bone regeneration were observed.
